You can conveniently order an implant via the online platform or use the PDF template to order by mail / fax. We will get in touch with you within 24 hours.

Request a case
Go to the Ad Mirabiles Online Service Link or download the Request Form and send it to Ad Mirabiles via regular post, email or fax.
2.
Design proposal
Ad Mirabiles sends the design proposal and the offer within 24 hours via Ad Mirabiles Online Service (automatic notficiations) or by email to you.
3.
Order and manufacturing
Once the design meets your expectations you can either order your implant via Ad Mirabiles Online Service or send the signed approval documents via E-Mail or regular post to Ad Mirabiles. Manufacturing is initiated immediately after ordering.
4.
Shipment
The finished implant will be shipped via international freight forwarder.
